It looks like the issue is related to having the plymouth-kcm package installed, however the Plymouth boot splash screens cannot be previewed outside of a TTY session, so the module in Plasma’s Systems Settings just shows image placeholders. You should still be able to select the themes, but you won’t see the boot splash you selected in action until you reboot.
