Look for a file called asound.conf
Also check with pavucontrol if a sink is created when playing audio in firefox. If not then check if the service is running:
systemctl status --user pulseaudio
So, I tried to use Spotify, then turn on Firefox, then pause Spotify without playing audio in Firefox. Shortly after I paused Spotify, audio shut down, which means it was having Firefox open, rather than playing audio in Firefox, that was triggering the issue

If a system is using software sound server PulseAudio, changing ALSA configuration /etc/asound.conf or creating custom user configuration .asoundrc should not be necessary
ALSA configuration to use PulseAudio as default sound server is usually provided by package pulseaudio-alsa with file /usr/share/alsa/alsa.conf.d/99-pulseaudio-default.conf:

It might also help to check audio stream information from Firefox and Spotify

So, I installed some updates (network manager &c), and tried to play audio simultaneously on Spotify and Firefox, and it worked (I don’t know if this is a temporary thing, for example, the possibility that if I pause for long enough, the error will re-emerge; so far, though, my attempts to pause one and play the other for a while, or pause both, have not caused any problems like they used to). My /usr/share/alsa/alsa.conf.d/99-pulseaudio-default.conf is as you shared, @nikgnomic. After posting this, I will move .asoundrc to a different file, and I’ll update upon reboot.
Now that audio is playing, my output devices tab looks like this (when it doesn’t work, the bar below the volume meter is always empty)

The problem re-emerged without a clear trigger; I had Firefox open (but wasn’t playing audio through it), then paused Spotify, then the error returned (the sign of this being that the headphones suddenly appeared muted and with a low volume in pavucontrol, but unmuting and raising the volume did nothing to allow the audio to play; Spotify could not advance).
I have yet to reboot, so perhaps deleting .asoundrc would work.

I rebooted, opened Firefox, and the problem recurred; deleting .asoundrc didn’t solve it
Does it only happen with spotify? If so, then I assume a bug there, but then there is not much we can do. Contact the spotify support in this case.
you can try to restart pulseaudio when it happens:
systemctl restart --user pulseaudio.service
It also happens with VLC; with Firefox open, the audio shuts off, but the video can still advance

Somehow, restarting PulseAudio a number of times got it to work. I’m not sure how.
I’ll update this thread in the future if the error recurs, or if this doesn’t fix it in future sessions.
The problem recurs after pausing; restarting PulseAudio, closing the audio-playing windows (Firefox tabs, Spotify &c) and starting it again allows one to hear something; older windows can’t continue to play.
I am having a similar problem with pulseaudio but I can get it to work usually by changing the Configuration from Analog Stereo to Digital stereo and back. I don’t know what that works, but it’s still pretty annoying. When this happens pacmd says suspend cause: APPLICATION
I’m using vanilla arch, might try a new profile as a troubleshooting step.
edit: i think this fixed it for me: i had an asound.conf file in /etc that i just renamed to asound.conf.old forgot i had done that at one point when I was trying to ditch pulseaudio. After doing that and rebooting, everything seems to work without having to change the configuration in pavucontrol!

Update: switching to Pipewire seems to have fixed the problem in toto.
